Services for Nutritional Consultations:
Please Note: Online Nutritional Assessment Questionnaire must be received 48 hours prior to your initial appointment (bring 3-day food journal in with you filled out).
Initial Consultation – (90 min) We’ll discuss your health history questionnaire, your health goals, your Nutritional Assessment Questionnaire, along with your current and past dietary habits and review your food journal. If consultation takes place in-office you will also take an oral pH test, and possible functional diagnosis exams. Can be in-person, over the phone, or virtual.
We’ll start the foundation for new dietary implementations and make supplementation recommendations, if necessary.
The Initial Consultation includes:
- Food Journal Analysis – Prior to this appointment, you will complete a three day food journal. During this appointment, we will discuss your journal. Maintaining a food journal helps you to become more aware of your eating and lifestyle habits.
- Personalized Nutrition Recommendations – I will give you personalized nutritional recommendations, along with both longterm and short term goals.
- Nutritional Assessment Questionnaire (NAQ) – Prior to your initial consultation, you will be emailed to confirm your email for the Nutritional Assessment Questionnaire. After confirming your email address, you will be emailed a link to the NAQ. You will then answer the 250 questions, which will allow me to recognize your specific imbalances and dysfunction.
- Nutritional Resources – These resources include recipes, handouts, links, and more that help you to achieve your nutritional goals.
Follow-up Sessions – (45 min) We’ll track your improvement and adjust your personalized recommendations depending on what’s working or what’s causing concern since your last visit. If you had a functional evaluation, we may re-test certain touch points to track progress. Can be in-person, over the phone, or virtual.
Nutritional Assessment Questionnaire (NAQ) Retake – Retaking the NAQ allows us to view your progress.

Please cancel at least 24 hours prior to your appointment. Failure to do so will result in a full cancellation fee.
Payment will be due at time of treatment. We accept credit cards or cash.
Insurance may cover acupuncture, but does not cover nutrition. Please call ahead to find out if you are covered for acupuncture.
We are distributors for several high quality supplement companies that we trust. Supplements may be purchased through our website under the online store or by signing up for a Biotics account through Maureen Tetelman distributor name.
Please ask for an instructions sheet for ordering at the front desk. You may also purchase supplements on your own.
